A.A. Andreyev, I.M. Sayfutdinov, A.Y. Anisimov

Abstract. Infected pancreatic necrosis is accompanied by development of morphological changes in the walls of portal vessels which result to thrombosis. Due to this fact 45 patients are investigated to rate the effciency of endoultrasonography in the diagnostics of portal vein thrombosis.

Key words: portal vein thrombosis, endoultrasonography.

PDF downloadPainless myocardial ishemia as the variant of the clinical current of ischemic heart disease

R.A. Galyavi, O.YU. Mikhoparova, L.I. Gornayeva, E.B. Frolova, N.B. Amirov

Abstract. To tap painless myocardial ischemia by means of ECG monitoring at patients from the proved ischemic heart disease and to establish sexual differences. To all patients defned the total cholesterol, spent the echocardiography and ECG monitoring. The general prevalence silent myocardial ischemia has made 10%. The share of men at which have been taped silent myocardial ischemia has made 47% (14 persons) Of 30 persons, women of 53% (16 persons). Middle age of men (52,6±6,04) years, women (60,9±12,2) years. At men TC (4,6±1,1) mmol/l, at women — (5,7±0,9) mmol/l (р <0,01). Emission fraction at men (55,6±13,0)%, (66,1±4,1)% at women (р=0,005). Amplitude of depression ST at men (1,3±0,4) mm, at women (1,1±0,3) mm (р=n.s.). The interrelation between depression of segment ST and emission fraction — r =-0,42, p=0,03 is taped. Painless myocardial ischemia at men from the proved ischemic heart disease arises at earlier age, despite authentically lower value of the general cholesterol and emission fraction of a left ventricle. The amplitude of depression of segment ST has negative communication with global contractility of a myocardium.

Key words: painless myocardial ischemia, depression of segment ST, ECG monitoring.

PDF downloadThe mediko-social characteristic of sources of the tubercular infection and estimation of its influence on risk of disease of children in the tuberculosis focuses

A.V. Mordyk, M.A. Plekhanova, T.G. Podkopayeva

Abstract. The comparative analysis of the basic sources of an infection in 105 centers of a tuberculosis in which lived 200 children is carried out. The most part of tuberculosis diseased were from sociopathic families and represented the greatest degree of epidemiological danger for children living with them. The income on one member of a family at them was below a living wage, 2/3 patients suffered from alcoholism, everyone 3rd was released from imprisonment places. At patients from sociopathic focuses for the frst time revealed fbrocavernous tuberculosis with bacterioexcretion, authentically more often with drug resistance were registered. Also it is established that stay of children in any centers of tubercular infection extremely adversely, to isolate reliably the sick parent from the nidus in an antitubercular hospital it is not obviously possible because of unwarranted interruption of treatment. We assume, what exactly dissociation of contact of the child with a member of the family, tuberculosis diseased, and also the additional social help to sociopathic families, will lead to considerable decrease in children's disease in the tuberculosis focuses.

Key words: children, the focus of tuberculosis, a source of a tubercular infection.

PDF downloadGender characteristics of heart remodeling of patients with chronic cardiac insufficiency of ischemic etiology

A.P. Rebrov, S.N. Tolstov

Abstract. The research of gender features of heart remodeling was transacted of the patients with ischemic etiology. 39 patients was investigated (16 women and 23 men) with chronic cardic insuffciency I — IV after old myocardial infarction. All investigated women were in menopause. General clinical investigation was transacted with using electrocardiogram with estimating type of simulating of left ventricle of heart. Geometrical adaptation of heart was registered. One of them, concentric type of remodeling, was detected in preference among women, and eccentric type of remodeling with more evident systolic dysfunction was visualize among men. The certain infuence in processes of adaptive remodeling of left ventricle of heart on concentric type among women may cause overweight body, more cases and more duration of arterial hypertension in comparison with men. Among men on severity of cardic insuffciency and character of myocardium damage infuence more frequency of repeated myocardial infarction, prevalence of smoking in studied groops, prevalence of men with normal weight, and previously eccentric type of left ventricle of heart with lowerd EF.

Key words: chronic cardic insuffciency, gender difference, remodeling of left ventricle of heart.

S.D. Mayanskaya, E.V. Malysheva, E.B. Frolova, O.Yu. Mikhoparova, L.I. Gornayeva

Abstract. There are different ways of combination antihypertensive therapy. In this case, drugs can be given consistently and gradually titrating their dose. After achieving the goal blood pressure level chosen combination of antihypertensive drugs can be used for long-term maintenance therapy of arterial hypertension. Therefore, the fxed combination of antihypertensive drugs with improved dosage forms are particularly important for rational treatment of arterial hypertension. Therapeutic effcacy of such drugs most effciently assessed using ambulatory blood pressure monitoring.
Key words: fxed combination, ambulatory blood pressure monitoring, effective antihypertensive therapy.

FOR GENERAL PRACTITIONER

PDF downloadProspective trends of digital (numeric) radiography

I.I. Kamalov

Abstract. Due to the fact that modern X-ray diagnostic equipment is completely converted from analog to digital production of images, there is a necessity to inform physicians about the prospective trends of digital (numeric) radiography and other methods of X-ray diagnosis. The principal distinctive mark of digital radiography and its advantage is the separation of the process of image forming on different stages, as well as qualitative improvement in spatial and contrast resolution of the images. Thus, the advantage of digital radiography over the conventional display-flm-type radiography is superior visibility of minor differences in contrast and a much greater exposure width, that reduces the number of defective images and reduces the radiation dose.

Key words: digital (numeric) radiography, perspectives of development.
