ARTERIAL HYPOTENSION IN CLINICAL PRACTICE

Abstract. Arterial hypotention is frequent and clinically significant disease. It develop most in young age and in elderly may cause cardiovascular diseases. Article describe classification, clinical symptoms, development, diagnosis and treatment of arterial hypotention. Current theories of pathogenesis of arterial hypotention are also present.

Abstract. Proposed injection treatment osteoarthroses based Plasmolifting method stimulate the regeneration of tissues with the use of autoplasma containing blood platelets. Methods of treatment with the use of platelet autoplasma in the treatment of osteoarthrosis of this technology is easy to use, does not require sophisticated equipment can take the pain, improve joint mobility, to promote processes of restoration of cartilage, bone and soft tissue of the joint, to prolong the period of remission of the disease.

ASSESSMENT OF THE FUNCTIONAL CONDITION OF THE ORGAN OF VISION AT THE STAFF OF DEPARTMENT OF INTERNAL AFFAIRS AFTER PSYCHOEMOTIONAL BURDEN

Abstract. Research objective — studying of functional violations of visual system at the staff of the law-enforcement bodies which have undergone influence of the psychoinjuring situation. Material and methods. Oftalmologicheskty examination of 79 male employees of Department of Internal Affairs aged from 24 till 49 years before business trip to North Caucasus region is conducted. For comparison of available pathology of an organ of vision the group arriving on service in Department of Internal Affairs on 1—2 groups of mission (80 people) aged from 21 till 39 years (on the average 29,1±5,1), comparable on a floor and age to the first group was selected. Results and their discussion. Certain distinctions in a condition of a refraction — prevalence of a miopichesky refraction at arriving on service in Department of Internal Affairs, prevalence of an emmetropichesky refraction at the staff of Department of Internal Affairs are revealed. 24% of employees after business trip showed complaints to visual fatigue, periodic dyscomfort, “fog” in eyes, and also as a separate and actual type of eye pathology the retina angiopatiya which frequency of emergence tends to increase with age, the frequency of business trips and duration of professional activity is revealed. Conclusion. Psychoemotional loadings at the staff of the Department of Internal Affairs who have come back from business trip, lead at part of patients to development of an asthenopia and a retina angiopatiya. With again arisen violations from an organ of vision in-depth study by doctors of other specialties is shown to patients.

INCREASE OF EFFICIENCY OF THERAPY OF ADDICTIVE DISORDERS BY THE ORGANIZING TREATMENT IN THE GENERAL MEDICAL NETWORK
Abstract. Aim. In article the model of addiction treatment, including involvement of general practitioners is analyzed It is include consultation, short-term interventions and pharmacotherapy of alcoholism, tobacco dependence and drug addiction. Material and method. Analytic. Results and conclusion. The conclusion that the inefficiency of contemporary addiction treatment model show that it need changes of a paradigm is drawn. Introduction of new model is capable to expand availability and a demand of such help, to attract the greatest possible number of the needing in treatment programs and improve quality of assistance to the persons being in general medical practice.
Abstract. Improve the results of surgical treatment of patients with intra-articular fractures of the tibial plateau submersible designs. Materials and methods. The study included 12 patients, aged 33 to 68 years (7 men, 5 women) with fractures of the tibial plateau. Structure fractures 3 patients — with Impress posterior medial condyle, 3 patients — with Impress anterior lateral condyle, 6 patients with a fracture of the tibial plateau of the two condyles. All patients was performed osteosynthesis submerged structures (reference condylar plate simulated condylar plate with angular stability) external fixation. Accuracy control of intraoperative repositioning carried out by visual inspection and medical imaging. The observation period ranged from 5 months now. Results. Using a submersible mellokonstruktsy and Ilizarov led to precise anatomic and stable fixation of bone fragments reposition the tibial plateau. Simulated condylar plate with angular stability allowed reposition and fix the bone fragments from different angles, creating a stable and rapid fixation of fracture lines, fuller exact reconstruction of the articular surface of the tibia plateau. The use of plates with polyaxial locking especially paid off in the osteosynthesis of fractures of the back or edge of the anterior tibial plateau, especially the introduction of the subchondral tibial bone to prevent the repeated displacement of damaged areas plateau in the development of movement in the knee joint in the postoperative period.

Abstract. Development of a variant of the perirenal novocaine blockade with control sites of injection and the duration of his stay in the tissues with a combination of sonography. Confirmation of the necessity and effectiveness of the blockade for the prevention of postoperative illness. The blockade improves microcirculation, dramatically phase exudation time reduced, reparative-proliferative processes stimulated, the frequency of inflammatory complications in the postoperative wound reduced.

LABORATORY DIAGNOSIS OF DISORDERS OF LIPID METABOLISM

Abstract. It is known that the most important risk factor for the development and progression of various diseases associated with atherosclerosis, are disorders of lipid metabolism. Timely and correct diagnosis and interpretation of the altered lipid profile, assessment of related risk factors — necessary conditions for the organization of a rational and effective prevention of major cardiovascular disease.

DRAVET SYNDROME

Abstract. Dravet syndrome is an autosomal dominant epileptic encephalopathy of childhood, which is caused mainly by SCN1A- and PSDH19-mutations. Although Dravet syndrome is well recognized, the causes of acute encephalopathy are still elusive, and reported data on ictal electroencephalography (EEG) and structural brain abnormalities scarce. SCN1A-mutations were found in 25 patients (83%). Two SCN1A mutation-negative patiens had chromosomal translocations involving chromosomes 9 and X, and one had a mutation in PCDH19. Prolonged seizures were associated with acute encephalopathy in three SCN1A-mutation-positive patients. One showed evidence of a significant hypoxic-ischemic event during status epilepticus. The over two demonstrated new persistent neurologic deficits postictally; they both carried heterozygous POLG variants (p.Trp 748Ser or p.Gly517Val). Hippocampal sclerosis or loss of gray-white matter definition in the temporal lobe was observed in 7 of 18 patients who had MRI after age 3 years (39%). Motor seizures were recorded on video- EEG for 15 patients, of whom 12 were younger than 6 years at recording; 11 patients (73%) showed posterior onsets. Our data imply that a heterozygous X and 9 translocation and rare POLG-variants may modify the clinical features of Dravet syndrome. The latter may increase susceptibility for acute encephalopathy. Temporal lobe abnormalities are common in patients imaged after 3 years of age. Focal seizures seem to localize predominantly in the posterior regions in young children with Dravet syndrome.

OARDIOOEREBRAL SYNDROME. THE MODERN CONDITIONOF THE QUESTION

Abstract. The purpose of the study. Describe the main clinical manifestations in patients with combined lesions of the coronary and cerebral arteries. Material and methods. In a review of published data on the prevalence and the main clinical manifestations in patients with combined lesions of the coronary and cerebral arteries. Results. The basic cardiocerebral syndrome clinical manifestations depending on the degree of coronary and cerebral arteries. Conclusion. Given the high prevalence of associated lesions of the coronary and cerebral arteries, and requires further scrutiny.

Abstract. The dynamics of number of staff of Internal Affaires office in the Republic of Tatarstan is analyzed in the article. The problems, tendencies and the consent of all the decisions made by the Military-Medical Committee in 2008—2012 are defined. The comparison strategy is used in the article, the situation in the Republic of Tatarstan is compared to the situation in other regions and the situation in Russia in general. The directions of the further amendments of Military-Medical Committee are being determined.
